How It Works
The HEARify Health Plan is a two-phase program designed to make hearing care interactive and engaging. Phase 1 is available to all patients starting 4 year treatment plans at our clinics.
Phase 1 – The Game Card
- At your first follow-up appointment, you’ll receive a HEARify Health Plan Game Card.
- Your audiologist will give you a personalized “prescription” with small, achievable tasks to complete before your next visit.
- You’ll earn points for each completed activity. The more points you collect, the better your hearing health rating.
- At each appointment, you’ll be awarded a badge or certificate of achievement for your progress.
Phase 2 – The Digital Experience
- We’re taking the HEARify Health Plan online!
- You’ll be able to track progress, earn badges, and compete on a leaderboard.
- Participate in weekly and monthly challenges for extra motivation.
- Connect with a community of like-minded individuals to share tips and encouragement.
